Contact/Request Forms

Online forms are fast replacing an email address listed on "Contact Us" pages. Spammers are getting smarter and are automating their email gathering tools. With your email address exposed to the world, you could very easily end up on a spammers list.

Your website is built with a contact form that is customized to the information you want to see when someone is asking for information about you or your practice. It could be as simple as a name, phone, email address and a comment box. You could also ask for a mailing address or insurance information. The forms are built to your specifications.

A contact form also protects your email address from spammers because the email is created after the user submits the online form. This also gives us the ability to route that email to anyone on your staff or even several people. You can have the information emailed to a specific person based on the choice you give the user. For example, if the user is contacting you about a billing question you can have the email routed to your accounts staff member. You can see this in action on our contact page. We route this email to different email addresses based on which subject from the dropdown field you would pick.

Web Template

As part of your basic package you will pick a web template that we will start with to build your website. You may select a template from the Package Templates page at no additional charge or one from an additional selection for an additional $500. You are sure to find one you like from these choices.

We then take the template you selected and start the customization process adding your name, images that you supply (optional) and navigation specifics.
